Our staff includes Registered Nurses (RN), Licensed Practical Nurses (LPN), and Home Health Aides (HHA) whose focus is to provide one-on-one support to meet our clients’ needs. Personal Health Care has the experience and ability to provide nursing support for nearly any person requiring healthcare assistance in their home. We provide comprehensive case management for each of our clients.
Our Multi-Disciplinary Healthcare Team Will:

One case manager to handle all of the client’s needs with oversight from the Director of Nursing.
