CULTURE OF SAFETY AT PERSONAL HEALTH CARE

At Personal Health Care our Culture of Safety is a just, reporting and learning culture where all employees feel safe raising questions and concerns and reporting safety events in an environment that emphasizes a nonpunitive response to errors and near misses. Our team regularly collects information from our employees and patients to learn from errors and successes, and openly shares this data and information to apply best practices and improve work processes and patient outcomes.

What to do when something goes wrong?

  1. Contact Chief Executive Officer (CEO), and Risk Manager, Ryan Muir at: 610-933-6130 ext. 10, or rmuir@personalhealthcare.org to report the event.
  2. In collaboration with Risk Management and the clinical team develop a plan to address the issue and correct the error.
  3. Know that we cannot always undo patient harm, but we CAN respond with our core agency values of Caring, Integrity, Excellence and Safety.

  • What happened
  • How it happened
  • What we are doing to prevent this type of event from happening again

Despite our best efforts and intentions, adverse events sometimes happen:

  • Report an adverse event immediately
  • Participate fully in the investigation
  • Seek or provide emotional support for all members of the clinical team involved in the event

Excellence requires courageous conversations.

We do the right thing

Start by contacting the Risk Manager for support and guidance. In collaboration with the clinical team, we will approach the patient and the family and:

  • Acknowledge what happened
  • Be empathetic
  • Explain what happened
  • Follow through with the family

Integrity begins with an apology.

We do the safe thing

Personal Health Care is concerned about the safety of our patients, their families, and our employees:

  • When the investigation is complete, use the lessons learned to avoid repeated harm and hold ourselves and others accountable
